Important Announcement

Courtyard surgery has been approved as a Training practice for GP registrars from the Mid Sussex GP Training Scheme.

Following our announcement earlier this year regarding Riverside Surgery moving premises, a date has NOT been set for the move. Riverside patients should continue to attend their appointments at the Riverside Surgery. Further updates will be posted on our website.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Passport signing
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Private Shingles Vaccinations

Courtyard Surgery can offer the Shingles vaccination to any person over the age of 50 not registered at our surgery. These are now available for our patients who are not in the relevant age band of 70, 78 and 79 years on the 1st September 2016.

The surgery cannot vaccinate its own registered patients against shingles if they are not in the relevant age band.

The private vaccine cost £200.00 and will be given at Park Surgery, Horsham and we will see Park Surgery patients in return.

If you are interested in receiving this vaccine privately please download the ‘shingles vaccination consent form’ and the ‘patient information leaflet’.

Please complete and sign the consent form and take it with your payment to Park Surgery. On receipt of payment Park Surgery will make you an appointment for your vaccination.

Park Surgery will advise us that you have booked for a vaccination and will request a copy of your summarised medical notes which will comprise your current medication, allergies, recent vaccinations and current major illnesses.
