Once you have rgistered you will be sent a confirmation text message as well as a link to any suplimentary questions that we may require answers to so we can complete your registration.

If you would like to register in paper format ,please print and complete the following forms and return them to the surgery:

GMS-1 Registration Form

New Patient Questionnaire (16yrs+)

New Patient Questionnaire (15yrs and under)

We also require the Red book/record of immunisations for any children under 5

Please note: if you complete a paper form you will need to bring your completed forms with identification (see below) to the surgery in order for us to complete your registration.

Identification

You will also be asked for identification to prove your identity plus evidence of your address in Horsham. Please note we will photocopy these documents for our records.

Acceptable forms of identification are:

  • Photo Driving license/passport/birth certificate
  • Bank or building society statement in your name showing your Horsham address
  • Utility bill in your name showing your Horsham address
  • Lease/tenancy agreement/rent book

If you are unable to produce identification to prove your identity or your address in Horsham we may not be able to register you.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.

Linked profiles in your NHS account

 Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
