If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

If you’ve been off sick for more than 7 days

To request a sick note:

  • If you have been off for more than 7 days and this is your first sick certificate being issued by the practice for this episode of sickness you will need to call the surgery for an appointment. 
  • If you are looking for a sick certificate extension you can submit your request via E-Consult or ask for an appointment  via our Patient Services Team. 
  • You will need to ensure you support the GP with dates of your sickness to ensure your certificate is correct. 
  • Please note we do not issue fit to work notes.
